24 Family Care is an NDIS provider supporting people with disabilities across Victoria and South Australia. Its team builds a personalised support plan from the first conversation, drawing on the person's NDIS goals to tailor support that helps them live more independently and take part in their community. Services cover assistance with personal daily activities, help with daily tasks in group or shared living arrangements, help around the home such as cleaning, laundry and meal preparation, development of life skills such as budgeting, cooking and public transport, and assistance with travel and transport.
24 Family Care also supports people through life stage transitions, helps them try new and enriching activities in their community, and offers community participation and group or centre-based activities. Support is delivered with an emphasis on person-centred care, respect and dignity, inclusion and collaboration with clients and their families.
