121 Care was started by people with disability for people with disability. Before the NDIS, 121 Care's focus was and always has been, choice and control for our clients. Prior to engagement, potential support workers interview with the client and their guardians to ensure there is a good fit. It is important to us that our client services team members have the required registrations, skills, experience and training relevant to the clients support needs.
Our supports and services include the following: • Client Services - in-home and personal supports, cooking, housekeeping, meal preparation, overnight stays, transportation, holiday supports, overnight stays, access to community activities and social services. • Support Coordination – consider your support coordinator as your personal coach and mentor, there to help you get the most out of your NDIS plan. Our support coordinators will work with you to achieve your goals as well as build and maintain a resilient network of formal and informal Supports.
• Supported Accommodation – we operate two residential facilities, one in the West End in Brisbane (Colville Lodge) and the other in Gympie (Colville House). Our highly trained team of support and community workers assist residents living with mental health conditions such as epilepsy, schizophrenia, bipolar disorder as well as intellectual disabilities and other mental health conditions. Services at both locations are 24/7 and include medication management, all meals, personal supports as required as well as community access.
• Supported Independent Living – we also operate a number of SIL homes in Redland Bay, Brisbane and the Sunshine Coast. Our shared house model is designed to nurture, engage and teach our residents skills they will need to live life as independently as possible. Our trained and experienced team of support and community workers offer assistance with personal supports, daily and domestic tasks, development of life skills, and community participation access.
For our clients in supported independent living residences, we introduce potential housemates prior to placement. No one wants a housemate that they don’t get on with, right? • Allied Health – on the Sunshine Coast and Gympie, our allied health service for people with disabilities is offered through Suncoast Integrated Therapies (SIT). Services include occupational therapy, behavioural supports, cold laser treatment, functional ability assessments and reports.
SIT also offers the MOllii Suit, a therapeutic assistive devise approved by the NDIS to assist people with muscle spasticity that result from an acquired brain injury, stroke, cerebral palsy, spinal injuries, multiple sclerosis and other neurological injuries. MOllii also assists with mobility issues, other neurological injuries. It is also known to assist athletes improve sports performance.
