We understand that plans can be quite difficult to read. Our team have expert knowledge of the NDIS meaning we understand what can be spent within each budget, and we know how to creatively get the most out of your plan. Working together At Melbourne Support Coordinators, you are our priority, we will work with you to achieve your goals, build your skills and independence and ensure that you have total choice and control of your supports and services. We will also work with your family if requested, your providers and carers that support you, to ensure that your needs are met, and you are receiving quality support and value for money. Support Coordination can be separated into three stages of working together. 1. Contact us to begin working with your Support Coordinator Whether you’ve just received your NDIS Plan, are half-way through your yearly funding or nearing the end – our Support Coordinators are here to support you in any stage. In our initial meeting your Support Coordinator will meet with you, either in person or online, the choice is yours. This proposed plan will include: A budget for the use of Support Coordination and how your Support Coordinator will support you throughout your plan A budgeted plan for the supports you currently receive Opportunities of how you can spend your NDIS funding Risks if you are about to exhaust funding for supports before your NDIS plan end date How we will resolve any highlighted issues together How we will advocate for you throughout your NDIS journey An outline of how we will report write for you How we will report to the NDIA on your behalf. 3. Review and Report Writing Two months before your plan review, your Support Coordinator will meet with you to discuss your year in review, if you have achieved your goals, what you would like to focus on for the year ahead and what further supports and services you require within your next NDIS plan. In preparation for your plan review, your Support Coordinator will: Report to the NDIA with case notes of your year, what further supports you require, and what you would like to focus on for the year ahead – this will be guided by you Collate and submit all assessments completed by Allied Health Professionals within the last two years Organise any final assessments that you are eligible for in preparation for the plan review Help you prepare for your plan review meeting and accompany you if you choose.
