Our services Assist Personal Activities provides support to everyday living tasks that you might find you need support with. We are able to assist and/or supervise personal tasks of daily life to enable you to live as independently as possible. These individual supports can be provided in a range of places, including but not limited to your own home. Supports could include for example: cooking shopping attending medical appointments in home support – lawn and garden maintenance Assist-Life Stage, Transition provides short and long-term supports that focus on strengthening the participant’s ability to coordinate their supports, and to assist them to live at home and participate in their community through services such as: support connection coordination of supports assistance with accommodation and tenancy obligations life transition planning including mentoring, peer support and individual skill development, and assistance with decision making, daily planning, budgeting. Daily Tasks/ Shared Living provides support with everyday activities and household tasks. Having support with everyday tasks such as getting ready for the day or cleaning the house allows you to live independently at home with assistance from our caring and experienced staff. If you are an NDIS participant and have been approved for the support category ‘Core Supports', you are able to access supports for daily activities and household tasks using your NDIS plan Supports Daily personal activities. We provide assistance with a range of everyday activities in the comfort of your home, supporting you to learn the skills needed to be as independent as possible. We are also able to provide these supports to people with complex needs. Personal activities can include: showering or bathing dressing grooming personal hygiene going to the toilet getting in and out of bed help with eating moving around the house Assistance with daily life tasks in a group or shared living arrangement If you live in house with other people, we can also help you with everyday tasks, supporting you to develop the skills you need to live as independently as possible. We can also provide respite care, day or night, to allow your carer to take a break, study, work, or go to appointments. Household tasks Having a clean, well maintained home is essential for your health and wellbeing. If you have trouble with household tasks, we can support you with: cleaning laundry dishwashing spring cleaning house maintenance, such as window cleaning preparing meals for you to cook Group/Centre Activities. We can provide and support Group and Centre Based Activities, including assisting clients to participate in the group-based community, social and recreational activities. Participate community Helping you to participate actively in community, social and civic activities will include supporting you during these activities, and developing your ability to take part in these activities. Helping you find activities in the community might include helping activity providers understand we are all different and have different needs. Providing you with support to find the activities you enjoy and helping activity providers listen to you and and respond to your needs may be important in allowing you to play an active part in your community. Personal Activities High means making routine and personal tasks both manageable and achievable. As part of our personal high activities’ assistance, we give flexible support tailored to meet your necessities and requirements. If you require temporary home-based care to assist you or your family or require more extensive and long-term personal activities assistance due to chronic disability, you can trust us with that. You will be helped by a worker with additional qualifications and knowledge that is relevant to your complex needs. Our individual support can be provided in a range of surroundings including, but not limited to, your home. By working alongside you in an environment that best suits your needs and lifestyle, we can make a strong relationship in a at ease setting to ensure you’re getting the most from the support provided.
