TESTIMONIALS 'I have found My Health Story to be very effective and useful to assist in accessing important health information, records and doctors letters all in one location.' – Care Manager Let's Get Care 'My Health Story, and it was definitely useful to have a quick overview summary of the rehab and therapies she had received to date before seeing her.' –Physiotherapist 'I've got a serious autoimmune issue which is my primary focus at the moment & I've just gone through & used the app to centralise all the info. On a personal level, this app is what I need to easily manage my health conditions.' – MHS Subscriber Nerita L 'A great resource for people living with chronic pain! Somewhere to safely track your medical appointments, results, experiences, goals, activity and then share with your health professionals making appointments more efficient and less traumatic.' – MHS Subscriber Mandy M I work with an elderly client. I was very unsure of myself and I’d I was getting things done to her liking and unsure of different moods and behaviours so I spoke to her daughter. She gave me access to her mothers My Health Story profile. Upon reading it so many things made sense. I gained a huge understanding of any moods she may be experiencing (so I know longer thought it was something I was doing) I also had a thorough understanding of her health history and triggers of pain. My work with the client improved immediately due to all the information I received in her my health story profile. I’d highly recommend this website, especially for those with complicated or extensive medical histories. It’s a game changer. – Personal Carer Sam M

From the provider: P.E.A.S. (operated independently by Catherine Pourreau) was born out of a strong desire to support and advocate for people with disability, to become recognized as invaluable participating members of the work-force and of their community. The aim is to help individuals express more fully their talents, their positive spirit and their creativity. Non-for-profit organisations that service or employ people with disability (and/or mental health issues) often run on a limited capital and rely heavily on carers, volunteers and government support to operate. As a practicing counseling psychotherapist, an art therapist and a somatic practitioner, Catherine has worked with individuals with different types of disabilities and levels of impairment, in private practice and in non-for-profit organisations that offer day programs and employment (Australian Disability Employment Organisations - ADEs). It is clear that a collective unmet need for specialized counseling/coaching support is significantly limiting the potential of these organisations. My role is therefore to provide counseling and mentoring support to individuals who want to create a richer, more independent life for themselves, but need extra support with: strengths-building, identifying their purpose and direction, increasing their motivation, participation and/or productivity. Catherine's skills lies in inspiring and motivating people, and helping them to orient their efforts towards reaching desired future goals. P.E.A.S. Services offered in the metropolitan Melbourne area include: psycho-emotional assistance to young and older adults with disabilities and/or mental health issues that plan to study, are already studying or are working. individual counseling and mentoring support for individuals during periods of change and personal growth. psycho-emotional support for parents and carers. management consultancy. CONTACT P.E.A.S. @ info@psychoemotional.com.au for inquiries and referrals.
