Zan Health Care is a registered NDIS disability support provider based in Cranbourne, in Melbourne's south east, supporting participants across Victoria. Registered with the NDIS Quality and Safeguards Commission until April 2028, the team delivers person-centred support designed around each participant's goals, choices and circumstances, with a focus on helping people live independently, confidently and with dignity. Supports cover assistance with daily personal activities such as bathing, dressing, grooming and mobility, and support to build everyday living skills.
In-home help includes cleaning, cooking, laundry, grocery shopping and light gardening. Participants are supported to get to appointments, work, school, shopping and social activities, and to take part in community life through individual and group activities that build skills, confidence and social connections. Innovative community participation supports help participants explore new opportunities and engage in activities that match their interests.
The team describes itself as compassionate, skilled and culturally sensitive, working alongside participants, families and communities. The office is open Monday to Saturday from 9:00am to 5:00pm and is closed on Sundays and public holidays. New participants start with an enquiry, followed by a personalised plan and then ongoing support.
