Yarra Valley Support Coordination is a support coordination provider based in Lilydale, Victoria. Established in 2019 by Sue Hayden after two decades in disability support, including work as an NDIS Local Area Coordinator, the organisation helps participants understand their NDIS plans, choose and negotiate with providers, track budgets and prepare for plan reviews. Specialist support coordination is available for people facing complex circumstances in health, education, housing or justice.
Services also cover support work, supported independent living, psychosocial recovery coaching, community and social activities, and home and yard maintenance or cleaning. The team includes an operations manager, scheduling coordinators, client intake staff, support coordinators and support workers.
