Verbinding is an independent NDIS registered provider based in Melbourne, trading as Verbinding Support Coordination and formerly known as Hans van de Graaff & Associates. The organisation provides Support Coordination (Level 2) and Specialist Support Coordination (Level 3) to National Disability Insurance Scheme participants, with clients in the Melbourne metropolitan area and in rural and regional Victoria. Coordinators help participants connect with service providers and mainstream supports, manage those relationships, prepare progress reports and plan reviews, and set up provider service agreements.
