As a provider Synergy Support Coordination Services focuses on providing high quality Support Coordination with a focus on responsiveness. I am a Sole Trader and have 15 years experience working in a range of Social Work settings including housing, mental health, child and family services and homelessness. I have been working as a Specialist Support Coordinator for four years and have an excellent level of knowledge regarding navigating the NDIS and supporting participants to get the most out of their plan I am specialised at supporting complex participants with a range of disabilities including complex mental health, ASD, Intellectual Disabilities, children and young people.
I have strong care team coordination skills and am able to establish new care teams or re energise existing ones. My skill set has enables me to succeed in stabilising participants in crisis and then support them to use their plan to meet their goals. As a Support Coordinator, I prefer to work closely with clients and have regular contact to ensure momentum is maintained and goals achieved. To do this well, I need to keep my case load small.
