Quality Pathways is a South Australian NDIS provider run by the Costa Family Trust, offering support coordination and general support services. Participants work with a support coordinator to understand their plan, put it into practice and coordinate the providers and appointments it funds. Support connection and a higher level of coordination are both offered, the second for people with complex needs. The service also covers individual and community group activities, mentoring and peer support, advocacy, and help with lifestyle management and life skills.
Support is delivered in person around Adelaide, the Fleurieu Peninsula, the Adelaide Hills, the Mid and Lower North and the Yorke Peninsula, and remotely or online for families in rural and remote communities. The practice is led by a director with lived experience of disability and a Certificate 4 in Disability.
