My Plan Connect is a disability support provider established in 2020 by Jodie O'Bree and Michael O'Bree, working from Leeton in New South Wales and Swan Hill in Victoria. It delivers personal care and self-care assistance, nursing support in the home, domestic help with cleaning and household tasks, and short-term accommodation in a respite home. Support workers are matched to each participant from a team with varied qualifications, trades and life skills, and supports are built around the person's own interests, from one-to-one outings to group activities such as camping, dance classes and cooking.
The provider also runs School Leaver Employment Supports for students in their final year of school and myDRIVESCHOOL, an online driving simulation with graduated pass/fail learning modules for learner drivers.
