Living Our Lives is an NDIS registered provider supporting people across Perth and the South West of Western Australia. A small, dedicated team based in Perth, it offers Support Coordination through its Living Well Pathway, a structured six-stage approach in which one Support Coordinator gets to know a participant's goals, plan and funding, sets up and connects the right supports, and tracks progress from plan start through to reviews and renewal.
Find Your Pathway is a one-to-one, NDIS funded discovery service for participants who want to explore what comes next, whether that is employment, volunteering, work experience, education, mentoring, community connection, self-employment or a micro-business. The team also runs practical one-day Find Your Pathway workshops that help people explore their interests, build confidence and work out their next steps, designed for people from all backgrounds and stages of life, including young people, people with disability, people over 50 and people from culturally and linguistically diverse backgrounds.
Life Connect Navigator is the team's free online resource of how-to guides, reflections and curated links for NDIS participants, families and carers. Living Our Lives Pty Ltd is registered with the NDIS Quality and Safeguards Commission for Support Coordination and assistance with life stages and transitions.
