LinktCare is a disability support provider based in Werribee, the trading name of Thriving Alliance Pty Ltd. The team supports NDIS participants across Wyndham, Melton, Brimbank, Hobsons Bay and the inner suburbs of Geelong, delivering Core and Capacity Building supports at home and in the community. LinktCare works with plan-managed and self-managed participants and states that it does not support NDIA-managed plans. Twelve supports are delivered in house by the provider's own trained team.
Personal and daily living supports cover showering, dressing, grooming and toileting, high intensity daily personal activities, mealtime assistance and medication prompting, morning and evening routines, meal planning and preparation, and overnight self-care. Household tasks include cleaning, laundry, washing and ironing, and house or yard maintenance and mowing. Community participation supports cover social outings, events and cafe visits, group activities and hobby clubs, with help to build social and community confidence, and a capacity building program focused on growing social skills.
Travel and transport support includes trips to medical and therapy appointments, transport to work, study and activities, shopping trips and errands, public transport training and door to door assistance. Short Term Accommodation and respite give participants planned and emergency stays with personal care and daily support during the stay, and a break for carers. Support Coordination provides a named coordinator who explains the plan in plain English, gives budget guidance, connects participants with trusted local providers and helps prepare for plan reviews.
Psychosocial Recovery Coaching offers one-on-one coaching towards mental health recovery goals at the participant's pace. Skills, employment and life transition supports include skills development and training, life transition planning with mentoring and peer support, assistance with accommodation and tenancy obligations, and practical help to find and keep a job. Coverage takes in Wyndham City Council, City of Melton, City of Brimbank, City of Hobsons Bay and Geelong inner suburbs including Geelong, Geelong West, East Geelong, South Geelong, Newtown, North Geelong, Rippleside, Belmont and Herne Hill.
The office is at Level 1, 44 Watton Street, Werribee, with meetings by appointment. Office hours are Monday to Friday, 9 am to 5 pm, with on-call and WhatsApp support for participants outside those hours, and enquiries answered within 48 hours. LinktCare states that every team member holds an NDIS Worker Screening Check and police check and receives ongoing training in participant safeguarding, first aid and person-centred practice, and that it works within the NDIS Code of Conduct and the NDIS Practice Standards with feedback, incident reporting, complaints, privacy and confidentiality processes in place.
