Hearts to Homes is a registered provider for services under the NDIS (National Disability Insurance Scheme.) We provide a range of high-quality services in which we are able to specialise in supporting all those living with disabilities. Our focus is to cater to all individuals whether they require homecare services, socialising or supporting our clients to become more independent. All services can be tailored to our clients needs. Our team of compassionate and dedicated staff will assist in all ways possible and provide the adequate support in order for our clients to reach their NDIS goals.
We provide support work and assist individuals who are unable to live independently by offering them practical help they need as well as the emotional support. This includes personal hygiene, eating and drinking, use of aids and appliances, mobility and transferring etc. We also help with home and garden maintenance by assisting with find the right services needed. We assist with doctors, psychologist, and other specialist appointments and taking the client wherever they need to be on time. H2H provide workplace assistance that enables an individual to successfully obtain and/or retain employment. Support can be applied to any working age individual (including students reaching working age) with an employment goal.
Essential skilled staff can provide capacity building supports that are likely to lead to successful engagement in a Disability Employment Service (DES). Here at Hearts to Homes Australia, we offer effective community challenges and participation programs for all participants. Community participation is important because it helps participants build and maintain relationships and encourages them to build their independence. These programs include: • Recreation and leisure activities.
• Personal interest activities like art, cooking, volunteering, etc. • Visiting the local library. • Attending community events.
