Hearth introduced a fresh and innovative approach to disability services in Victoria in 2017, with person-centred, long term sustainable support. Central to our Hearth mission is enabling participants to realise their strengths and lead fulfilling lives, at home and in the community. So, we prioritise trust and a holistic approach which responds to individual needs, recognising that there are many aspects to life building and fulfilment. Hearth is NDIS registered.
The disability support we provide includes Support Workers, Allied Health and Positive Behaviour Support services across Metropolitan Melbourne. There are several aspects of the Hearth approach that distinguishes us from traditional providers – each very important to our vision to lead Australia in supporting individuals with a disability, lead their best life. They include our: - Relationship Manager role - Passion for Support Worker matching - Allied Health expertise - The Hearth Advisory Board which includes sector experts and researchers - Sector innovation agenda including Employment, life and transition Planning (My Pathway, Employment, Housing and General) - Strong central support team Our Allied Health services include: - Occupational Therapy - Physiotherapy - Speech Pathology - Positive Behaviour Support - Music Therapy Our Support Services runs across shifts from a few hours to 24/7 support at home and in community, and includes: - Assistance with Daily Life - Self-Care Assistance - Supported Independent Living (SIL) - High Intensity Support including Complex Medical and Hospital Discharges - Companionship Building Strong Healthy Relationships.
