Future Focus Support Coordination is a support coordination practice of Future Focus Group, working with NDIS participants. The team delivers Level 2 coordination of support and Level 3 specialist support coordination for people with complex needs navigating mental health, justice, housing or drug services. Coordinators help participants connect with providers, manage budgets and build capacity, while specialists work to maintain safety and continuity of care. Support runs across Victoria, Western Australia and Queensland, meeting participants at home, in the community or at listed offices. Staff speak Cantonese, Mandarin, Vietnamese, Hindi, Gujarati and Spanish as well as English.
