Collective Coordination is an NDIS registered independent boutique support coordination and plan management provider. We are a dedicated team of professionals in the community services sector focused on helping people with disabilities to plan their support options and manage their NDIS funding. We are focused on providing individualised care, that is always based on evidence. We recognise that individuals function within family and wider systems and that these need to be considered in service provision.
This is why relationships and collaboration are at the centre of our work at Collective Coordination. Our support coordinators and spescialist support coordinators have allied health or nursing background and years of experience in the disability, health and mental health secotrs. Our recovery coaching is a partnership between learned and lived experience providing the best of both worlds to our clients. Our plan management service is boutique and due to this more personalised.
As a unique touch, all of our clients have access to a visit from our peer liaison worker. We aim to bring together best of modern technology and human touch in all service provision.
