We provide 2 Levels of Support Coordination services: a. Level 1 Support Connection This support is for participants who already have a deep understanding of their own needs, and have a matured care plan that they can confidently implement, but want to get the most out of their plan, through better connections with informal, community and funded support to pursue their goals. b. Level 2 Coordination of Supports (CoS) This support is for participants who are on their journey to further understanding their needs and are building up the skills needed to design and implement their own plan.
No matter your goal is to increase your capacity to maintain relationships, manage service delivery tasks, live more independently or be included in the community, our experienced Support Coordinator will work with you from start to finish, to ensure the best mix of supports are selected to suit your needs. Meals Services At CCA, we offer nutritious meals delivered to your door. We can deliver meals hot, chilled or frozen 7 days a week. Special dietary requirements menus are available.
