Social & Community Participation We provide support and assist you to access the community and engage in activities that you love. These activities can be in a group or just for you and a friend or individually. We can support your social activity and help make sure it is enjoyable. Getting out and about in the community will help you learn new skills such as how to confidently use public transport, explore new places, make new friends, and most importantly, do the things you love and enjoy!
And the best part is that you can access our Social and Community Participation services in your own home, in our hub, or out in the community, its completely up to you! Day Programs Our day program provides individuals with a disability the opportunity to engage in activities that interest them. We provide a relaxed, fun and social environment aimed toward facilitating the ideas and interests of everyone who participates.
Our disability programs can help you: - Gain life skills and build capacity for daily living, helping you be more independent in everyday life - Stay physically active while having an opportunity to get out of the house - Develop skills that allow you to find work - Try different hobbies and develop new interests in a friendly group setting - Achieve your own goals with plenty of support from a dedicated staff team - Make new friends and spend time with other people in a warm and inclusive environment and more...
Specialist Disability Accommodation We provide Specialist Disability Accommodation housing that enables people with extreme functional impairment or very high support needs to live comfortably and receive their daily supports. It can include shared supported accommodation, community residential units, or self-contained homes that are fitted with specialist housing solutions.
Supported Independent Living We provide SIL which is the assistance you receive from a support worker to live within your home. This support can extend to activities such as personal care, cleaning, physical support, cooking and other areas depending on your needs. This funding may apply to a combination of 1:1 supports, shared supports and irregular (unplanned) supports. Your SIL funding is independent of where you live. So whether you rent privately, live with others or in Specialist Disability Accommodation (SDA), your SIL funding is based around your needs, not your housing model.
Short Term Accommodation & Respite We provide Short Term Accommodation, including respite, is support for when you need to live out of home for a short period. Short Term Accommodation funding can be used for respite to support you and your carers. This gives your carers a short break from their caring role. This type of service can: - give you the chance to try new things - can be a place to make new friends or develop new skills - may help to maintain your current living situation by giving your informal supports a break.
At Australian Epic Health Services we are able to facilitate a smooth and flexible intake process allowing us to provide support in the face of sudden changes. Support Coordination Support coordination helps you to make the best use of your supports in your NDIS plan. Support coordinators help to identify and connect you to a range of service providers, help you to understand and implement your plan, and answer any questions you may have.
Support coordination also helps by empowering you to become confident in understanding how to work with service providers, resolve support issues that may arise and participate independently in the NDIS. Support coordinators can work with you to : - assess a participants NDIS plan to have a conversation about what is possible with the funding. - choose preferred options or providers - assist individuals to resolve any conflict or issues with service delivery.
- Provide support for individuals in times of crisis. - negotiate services and their prices, develop fair service agreements and create service bookings with the provider of your choice - negotiate services and prices as part of any quotable support - Resolving, changing or ending service agreements and helping individuals understand their responsibilities under them. At Australian Epic Health Services, we provide 3 levels of support coordination.
These are support connection, coordination of supports and specialist support coordination. You will need to have funding for this included in your NDIS plan, to access these services. Your needs will always come first. So we’ll also chat with you to make sure that our services are just the right fit.
