Every participant we work with gets a consistent, named support worker who takes the time to understand their goals, their personality, and the life they actually want to live. No rotating roster of strangers. A real local team who shows up, gets to know you, and stays. Our Services We provide three NDIS support worker services: Daily Personal Activities: Everyday personal care and daily living assistance delivered with dignity and at the participant's own pace. We focus on non-clinical, community-based personal care that helps people feel confident and comfortable in their daily routine. Independent Living Skills: Practical support to build real, lasting independence. This includes cooking, budgeting, using public transport, managing a home, and developing the everyday skills that give people greater control over their lives. Social and Community Participation: Getting out, trying new things, and building genuine friendships across Brisbane. We support participants to connect with their community, explore interests, and work toward meaningful social goals. We also support Short Term Respite (formerly Short Term Accommodation) for participants and families who need planned breaks. Who We Work With As a non-registered NDIS provider, we support self-managed and plan-managed participants. We work with people of all ages and across a wide range of disabilities and support needs. NDIA-managed funding is not something we are able to accommodate at this time. The Horizon Method Our approach is goal-driven and collaborative. We start by understanding what is getting in the way of a participant's goals, build a plan together, and work alongside allied health teams where needed.
