I tailor all care personally with a huge focus on independence and individuality. Not one care delivery will be the same - we are all unique after all. I bring bring a positive, upbeat attitude with a smile. I have 8 years experience in acute and trauma based settings and bring the following skills and capabilities: Clinical skills Wound care and management, enteral and parenteral nutrition administration (PEG, Mickey, TPN administration), management of colostomy's (stoma's), management of airways - Tracheostomy's etc, management of invasive devices such as PICC’s, CVL’s, portacath’s, urinary catheterisation, seizure management including monitoring, administration of medication and safe clinical escalation, Administration of IV medication, blood collection, cannula insertion, subcutaneous injections. Outside of hands-on clinical skills, my experience and scope allow for the following care provisions. These skills have been further enhanced by upskilling through achieving my Master of Nursing which focused on proficiency in care plans, care coordination and specialised care delivery: Facilitation of care plans provided by an external provider, for example, an OT, physiotherapist, dietician, psychologist, promote adherence at a grass-roots level outside of sessions, provide suitable encouragement and clinical supervision where necessary, monitoring response (physical, emotional, psychosocial) to treatment and care plans and providing constructive communication to the provider to allow for care enhancement, delivering therapies (within Registered Nurse scope) recommended by an external provider. Developing nursing care plans for Enrolled Nurses and Assistant Nurses, evaluating existing care plans and identifying gaps in care or barriers with the aim to improve care delivery, assessing health needs and recognising appropriate referrals required to enhance a client's overall wellbeing, providing medication education and identifying and rectifying barriers for adherence, providing health education in alignment to a client's health literacy, management of chronic conditions through education and provision of care, appropriate documentation of assessment and care + more!
